LifeCare Health Partners Announces New Name for Texas Home Care Agencies

BeyondFaith Homecare and Haven Home Health unite under single brand

PLANO, Texas, Jan. 30, 2019 /PRNewswire/ — LifeCare Health Partners today announced 10 of its Texas-based home care providers will join under a new name: LifeCare Home Care, effective immediately.

Over the past nearly four years, LifeCare Health Partners, a leading national health care services provider, has expanded its post-acute services to include home-based care, intermediate skilled nursing, transitional care and behavioral health services. In 2015, the company acquired Haven Home Health, a multi-location home health care provider in the greater Dallas-Fort Worth metroplex area. In 2017, LifeCare Health Partners completed the acquisition of BeyondFaith Homecare & Rehab, with multiple locations across Texas.

Eight of LifeCare’s Texas-based home care agency locations were included on the 2018 HomeCare Elite list, which recognizes the top 25 percent of all Medicare-certified home health agencies in the country based on several performance measures, including quality of care, quality improvement, patient experience (HHCAHPS), best practices and financial management. The Graham, Texas, location achieved the highest-level honor, being named one of the Top 100 home health agencies in the nation.

The dedicated team of more than 270 professionals at LifeCare Home Care provide services including: skilled nursing, physical therapy, occupational therapy, speech therapy, enterostomal therapy, home health aide and medical social services. LifeCare Home Care also helps coordinate medical equipment, oxygen, infusion medication, pharmacy medication, community resources, hospice, caregivers/companions and physician house calls. The team specializes in wound care, enteral feedings, infusions and injections, disease management, ostomy care, catheter care, medication management, care education, and post-operative nursing and rehabilitative care. In 2017, BeyondFaith and Haven served nearly 8,000 patients across Texas.
