NEW YORK, Aug 23, 2012 /PRNewswire/ — Selfhelp Community Services and FEGS Health & Human Services, two leading New York based not-for-profit organizations providing a broad range of health and social services to seniors and others with chronic medical conditions, have joined together to establish SinglePoint Care Network, a care management company, in response to emerging changes in the managed long term care market resulting from Medicaid reform in New York State.

Through a strategic partnership with HealthPlus Amerigroup, one of the largest Medicaid Managed Care companies in NYS, SinglePoint will, pending approval by the NYS Department of Health, become the delegated care manager for many of the members of HealthPlus/Amerigroups’s managed long term care plan.

SinglePoint will coordinate and manage the care and delivery of services to eligible enrollees through the application of tools assessing client needs for long term care services and by actively managing appropriate long term care.

The IPA will also provide management and other shared infrastructure services to providers who join the SinglePoint IPA network.
